Student ID Photo Submission and Parking Permit Information Form
(Complete this only if you do not already have a Mercy Health Sciences University Student ID)

- A device with a camera is required.
- Computer users generally will need to select “Use Rear Camera” before taking your photo.
- iPhone users must install the Microsoft PowerApps app for camera access.
- Android user can either install the app or select “Continue in browser”.
- Log in using your Mercy Health Sciences University Microsoft 365 account (first.last@MercyHealthU.edu)
- Follow the on-screen instructions to take/upload your photo and enter your vehicle details. Make sure you check the boxes and select submit to see the final confirmation screen.
- You will be emailed when your badge is ready for pickup usually within two to five business days.
NOTE: If you use a computer select “Rear Camera” to enable viewing of your web camera.
